FORMANEK, Norbert Michael, age 72, passed away on November 3, 2013, in Nashville, TN. He is preceded in death by his parents, Stanley and Mary Formanek and sisters, Arlene Formanek and Theresa Cole. He is survived by his children: Michael Formanek and Michelle (George) Winslow, both of Ashland City, TN; granddaughter and great-grandson, Tiffany Killmer and Keaton Maxwell, both of Ashland City, TN; and sisters, Pat Mindzora and Linda Mosier, both of Cleveland, Ohio. Norm was born in Cleveland, Ohio and became a carpenter. He will be missed by all who knew him. The family will welcome friends at a celebration of life visitation on Thursday, November 7, 2013, from 5pm to 7pm in the Chapel of Cheatham County Funeral Home.
